Patient App Privacy Notice

This notice explains how Human Health handles the personal data of patients who use our patient app. It sits alongside our main website Privacy Statement and covers the patient app specifically.

1. Who we are and who is responsible for your data

Human Health Ltd (registration number 120483) is the data controller for your patient records. We decide what data is collected through the app and why.

Our patient app is provided to us by PracticeHub, operated by Zyricon Ltd, a company registered in Cyprus. PracticeHub acts as our data processor, which means they securely host and store your data on our behalf and only process it on our instructions. 2. The data we collect through the app

Depending on how you use the app, we may collect and store:

  • Identity details: your first and last name, date of birth and title.
  • Contact details: your email address, telephone number and postal address.
  • Appointment data: your bookings, appointment history and related preferences.
  • Clinical information: notes, assessments, health history and other records relevant to your care.
  • Messages and documents you send to us or share with us through the app.
  • Technical data: device information and app usage needed to keep the app secure and working.

We only collect what we need to provide your care and manage your appointments.

3. How we use your data

We use your data to book and manage your appointments, to provide and record your chiropractic care, to communicate with you about your care, to meet our legal and professional obligations, and to keep the app secure.

4. Our legal basis for using your data

We process your data on the following bases: to perform our care agreement with you, to comply with our legal and professional obligations, where you have given consent, and, for health information, because it is necessary for the provision of healthcare. Where we rely on consent, you can withdraw it at any time.

5. Where your data is stored and who processes it

Your data is stored securely by PracticeHub on our behalf. PracticeHub applies encryption, access controls, secure hosting and regular security testing to protect it. Any transfer of data outside the European Economic Area is covered by the safeguards required under data protection law, such as Standard Contractual Clauses.

6. Sharing with other parties

We do not sell your data. We share it only with PracticeHub as our processor, and with the sub processors PracticeHub uses to run the service. PracticeHub keeps an up to date list of these at https://go.practicehub.io/v3/trust-centre/subprocessors. We may also disclose data where we are required to by law.

7. How long we keep your data

We keep your patient records for as long as you remain a patient of Human Health. After your last appointment, we keep clinical records for 8 years, and longer for patients who were treated as children, in line with our professional and legal obligations. When that period ends, we delete the data or instruct PracticeHub to delete it.

While your record is held in the app, PracticeHub retains it on our instruction. When data is deleted from the live system, any residual copies held in PracticeHub's encrypted backups are overwritten within 35 days.

8. How to request deletion of your data

You can ask us to delete your personal data at any time. To make a request:

  • Email us at hi@humanhealth.je, or
  • Call us on 01534 747833, or
  • Speak to a member of the team at either clinic.

Please give us your full name and date of birth so we can confirm your identity and act on the correct record. Once we have confirmed your request, we will delete your data and instruct PracticeHub to remove it from the app, apart from any records we are legally required to keep. PracticeHub removes data from its live systems on our instruction, and residual backup copies are overwritten within 35 days.

9. Your other rights

You also have the right to access your data, to have it corrected, to object to certain processing, to restrict processing, and to request a copy of your data in a commonly used, machine readable format. To exercise any of these rights, contact us using the details below.

10. Children

Where we treat patients who are children, their data is provided and managed by a parent or guardian, and the same protections and rights in this notice apply.

11. Complaints

If you are not satisfied with how we handle your data, you can complain to the Jersey Office of the Information Commissioner, 2nd Floor, 5 Castle Street, St Helier, Jersey, JE2 3BT.

12. Changes to this notice

We may update this notice from time to time. The date at the top shows when it was last changed.
